Vitali-T Men’s Health Clinic, LLC, transitioning to DBA Vitali-T Health & Wellness Clinic
(“Vitali-T,” “we,” “us,” or “our”), provides this website and its content (collectively, the “Site”)
subject to these Terms and Conditions and applicable law. By accessing or using the Site, you agree
to these Terms. If you do not agree, you must discontinue use of the Site.
1. OWNERSHIP AND PERMITTED USE

by or licensed to Vitali-T and are protected by applicable intellectual property laws.
You may view, download, or print limited portions of the Site for personal, informational, and

Do not use the Site, email, text messaging, or the patient portal for a medical emergency. Call 911
or seek immediate emergency care.

error, omission, interruption, technical problem, or other consequence arising from your access to or
use of the Site or third-party content.
4. TEXT MESSAGE TERMS
By providing your mobile telephone number and consenting to receive text messages, you authorize
Vitali-T to send messages regarding:
Appointment confirmations and reminders
Scheduling and follow-up communications
Account and service notifications
Prescription, medication, laboratory, and refill notifications
Billing and payment reminders
Educational information

and communicate with you as permitted by law and our Privacy Policy.
Electronic communications are not guaranteed to be completely secure. Sensitive medical
information should be submitted through the designated patient portal whenever available.
6. MEDICATION DELIVERY AUTHORIZATION
When applicable, you authorize Vitali-T staff to accept delivery of medication from an authorized
dispensing provider on your behalf and provide the medication to you as prescribed, subject to
applicable law and clinic procedures.
7. USER CONTENT
If the Site allows users to submit reviews, comments, messages, photographs, or other content, you
are responsible for the content you submit.
You may not submit content that is unlawful, false, misleading, defamatory, threatening, obscene,
harmful, infringing, promotional, or invasive of another person’s privacy.
Do not publicly submit confidential medical information or personally identifying information.
By submitting content intended for public display, you grant Vitali-T a nonexclusive, worldwide,
royalty-free license to use, reproduce, display, distribute, and adapt that content for purposes related
to the Site and its services.
Vitali-T may review, remove, restrict, or decline to publish user content but is not required to
monitor all submissions.
8. LINKS TO THE SITE
You may link to the Site provided that the link does not misrepresent your relationship with Vitali-T,
imply an endorsement, obscure Site content, or display the Site within unauthorized frames or other
materials.Vitali-T may require any link to be removed at its discretion.

Vitali-T may update the Site or these Terms at any time. Updated Terms become effective when
posted. Continued use of the Site after an update constitutes acceptance of the revised Terms.
10.ENTIRE AGREEMENT AND SEVERABILITY
These Terms, together with the Privacy Policy and other notices referenced on the Site, constitute
the agreement governing your use of the Site.
If any provision is found unlawful or unenforceable, that provision will be severed, and the
remaining provisions will remain in effect.
